Eligibility Criteria for Himmat Card Registration

You must meet the following criteria to register for a Hemat card. 

• You must have a disability certificate approved by the Punjab Social Welfare Department. 
• You should belong to a low income family. You must not have a job. 
• You must have a National Identity Card. 
• You should belong to Punjab province.

Registration Himmat Card Through PWD App

If you want to get registered in Himmat Card and you have an internet facility then Punjab Government has launched a PWD app through which you can easily complete the registration in Himmat Card.  Steps to follow:

• Open the Play Store on your mobile phone. Now search for the PWD app in the search bar. 
• In front of you will come the PWD app which is developed by Punjab IT Board.
• Click and install the app and after successful installation create your account. 
• After creating an account, log in to your account. 
• After logging in you will be presented with a registration form, doctor appointment, and other options.  
• You have to click on the registration form.
• Now you have to enter your registration information in the form like your name, surname, address, phone number, email address, CNIC number, father’s name, and information about your disability.
• Apart from this you have to provide your education and income information.
• After entering all the information once make sure your information is correct then submit the form. 
• After submitting the form, book an appointment with your doctor.  
• Or else you have to provide a disability certificate from Punjab Social Welfare Department.
• Because it is to ensure that the right person will get Rs 10,500 from the Punjab government every three months. 
• After going through all these steps your registration process will be complete.

Registration Himmat Card Through DHQ Hospital

Apart from this, HIMAT Card also has an offline registration procedure where you have to visit the District Headquarters Hospital (DHQ).If you want to enroll for HEMAT Card through DHQ Hospital then you have to follow all the following steps:

• First collect the documents required for your registration like your CNIC, your disability certificate if you have it, and other information. 
• Visit your DHQ Hospital during working hours.  Make your appointment with the doctor there. 
• The doctor will check your disability status and if you are found to be disabled, you will be given a disability certificate from the Punjab Social Welfare Department.
• After getting the disability certificate you have to get the form for issuing a courage card from the DHQ hospital representative.
• The representative at the reception will give you a registration form. 
• You have to fill this form along with your personal information in the registration form.  
• After entering all the information, read the form carefully once to avoid any mistakes.  
• After that, hand over the form to the representative. 
• After submitting the form you will be enrolled in Himmat card and you will be provided Himmat card on the date fixed by Punjab Government.
